Financial Policies

Our office participates in a wide variety of traditional and managed care insurance plans, including Medicare. Please call the customer service phone number located on the back of your insurance card to verify participation in that specific plan.

Bring your insurance cards with you at each visit. We file claims as a COURTESY to our patients.

ALL CO-PAY'S are due on the day of each visit when you sign in. You may be asked to pay any deductible or outstanding balances on your account prior to being seen. We sign contracts with insurance companies and we must abide by their policies on the collection of co-pay's at each visit.

Please note: regardless of your insurance coverage, you are  responsible for making sure your medical bill is paid.

We accept MasterCard, Visa, Discover and Debit cards.

Uninsured patients-Self Pay:

We understand this is a difficult time for you. A $100 deposit is required the day of the visit. We offer a 25% discount for services paid on the same day.

We also offer payments plans.
